New Patients

Newcomers are registered with the practice - North House Surgery - but you can still ask to see the doctor of your choice. Obviously it will not always be possible to see that particular doctor because of training commitments or holidays, for example.

The reception team will be pleased to help you and answer any questions you may have about services available within the practice. They will also offer you an appointment for an initial health check with either a doctor or a nurse.

We also provide medical services for temporary residents within the area or patients requiring immediate treatment. Again, the reception team will help you to make use of this service.

Registration Forms

You will need to download both forms and return the completed forms to the reception team.

New Patients From Outside UK

In concise form and in a number of languages (click on the appropriate link to access .pdf), there is information about the role of the General Practitioner and how the primary care system fits into the overall NHS structure. There are also details on how to register with an NHS practice, how to access emergency services, and other useful information.
